Since Ration card is not being recognised as a valid residence proof, Pan card does not contain our residential address and Aadhaard card can not be utilised towards the proof for Nationality; Passport is the only solution and a single document which can be utilised as photo id, residence proof and to prove our nationality.
Married females need to keep ready the following documents if they are applying for passport in their new name.
- 1) Annexure D (Marriage Affdvt.)
Marriage certificate is not sufficient if it does not contain our name after the marriage. E.g. A girl ABC marries to a boy PQR; now if her name remains the same she will become APR. But if her name changes from A to say M then she becomes MPR. Now since you don’t have any document to show that you are MPR here comes the document Annx D into picture. It is a declaration stating that a Person ABC is married to the person PQR under Hindu Marriage Act, are living together as married couple since dated so & so and will be known as MPR henceforth due to marriagd. This affdaivit is required to be typed on a least valued stamp paper i.e.Rs.20/-, with one latest photograph of husband and wife together, signature of both and then required to get it notorised.
2) Annexure E(Sworn Affdvt.) This document is to be prepare to state that you are MPR previously known as ABC and henceforth you will abandon the use of your previous name ABC.
This affdaivit too is required to be typed on a least valued stamp paper i.e.Rs.20/-, need two witnesses who knows you and need to get this affidavit notorised.
*It is also stated that you need to change the name in the official gazzet and confirm the same by giving an advertisement in minimum 2 newspapers.
